BECOME A CORPORATE PARTNER

There are three ways to apply for or renew your partnership.

  • OPTION 1 (PREFERRED): Use our online application form (below) and complete your payment online via credit card. Your membership is active immediately after payment is confirmed.
  • OPTION 2: Download and complete this Corporate Partnership form, and then scan it and email it to our Treasurer. An online invoice will be generated and sent to you. Your membership is active after successful payment of the invoice.
  • OPTION 3: Download and complete this Corporate Partnership form, an then mail it to the address below with a check payable to the Upper Midwest Flute Association. This is the longest process and may result in delays in activation of your membership.
