There are three ways to apply for or renew your partnership.

  • OPTION 1 (PREFERRED): Use our online application form (below) and complete your payment online via PayPal. Your membership is active immediately after payment is confirmed.
  • OPTION 2: Download, complete and scan our offline Corporate Partnership form, and email it to our Treasurer. An online invoice will be generated for you via QuickBooks. Your membership is active after successful payment against the invoice.
  • OPTION 3: Download, complete, and mail in the Corporate Partnership form. Our mailing address is below, and all checks must be made payable to UMFA. This is the longest process and may result in delays in activation of your membership.
